Chelsea will celebrate winning the Premier League title when they take on Sunderland at Stamford Bridge this afternoon. 

Antonio Conte has handed a start to John Terry for what will be his final appearance in a Chelsea shirt as the long-serving skipper replaces Gary Cahill in the back three.

Thibaut Courtois starts in goal while David Luiz and Cesar Azpilicueta also line-up at the back with Cesc Fabregas joining N’Golo Kante in midfield.

Willian and Eden Hazard support Diego Costa in attack with Victor Moses and Marcos Alonso on the wings.
